Religious Education Registration 

1. print and fill out a registration form
2. attach your payment (cash or check) $50.00 for child $90.00 for a family of 2 or more children
3. attach a copy of your student's baptismal certificate
4. place in an envelope labeled "CCD registration"
5. mail or drop it off at the parish office: 420 Inman Ave. Colonia, NJ 07067

Self Administration of Medication: 

Any student who must carry medication or an epi-pen must submit this form yearly.
